Money is really tight this Father’s Day. I knew I wasn’t going to be able to go out and buy a gift for my spouse this year, but I wanted to give him something sweet and memorable. That’s why I decided to make this Father’s Day printable questionnaire to share with all of you!

How We Used This Father’s Day Printable Questionnaire

This year Moose is only 3 and hasn’t started writing yet. I decided to ask him each of the questions on this questionnaire and I wrote down the answers that he gave in each space. Let your little one draw a picture of their daddy in the circle at the top. I have decided to let him fill out this questionnaire for his daddy every Father’s Day so we can see how his answers change as he gets older. I’m going to put them in a scrapbook to look back on for years to come.
